Our outreach services provide tailored support hours that are based in the Individual’s home and out and about in the community. Our team devise a flexible, personal and outcome focused care plan that supports an Individual’s interests, wishes, needs and independent living skills. These may include support with daily living tasks, making and attending appointments, socialising, managing financial arrangements and pursuing interests and activities.
